DIHC Information & Documentation

Dudley Integrated Health & Care Trust (DIHC) is the body that has superseded Dudley CCG in commissioning services. The following is documentation required by them for community pharmacy to read and respond to. The Information Request form needs to be completed and returned to them at dihc.contracts@nhs.net

• The Sub-Contract Information Request form
• A copy of the 4 Dec 2020 Provider Engagement Session presentation including questions raised and the DIHC responses
• A draft copy of the Integrated Care Provider Sub-Contract
